At Lake Norman Orthopedic Spine Center our compassionate caregivers understand the challenges facing patients with spinal disorders. We treat each patient as an individual with unique needs and use a team approach to enable fast and effective diagnoses and treatment recommendations. We believe in educating the patient as much as possible on his or her condition and the available treatment alternatives.
Our staff is dedicated to providing information and answering questions so that patients can take an active part in their own care decisions. Our staff is caring, accessible and knowledgeable. We are available 24 hours a day, seven days a week for urgent issues.
Appointment Preparation
Thank you for scheduling an appointment at Lake Norman Spine Center. In order to better serve you and save time, you may download the forms located in the Forms section of this website, fill them out, and then bring the completed forms with you to your first appointment. Otherwise, please remember to arrive 15 minutes prior to your appointment to complete the new patient forms.
What to Bring
- Completed new patient packet.
- Valid identification
- Insurance card
- MRI and/or X-ray films or CD and any records pertaining to your neck/back injury or condition.
- List of all medications – including over the counters and herbals.
- Co-pays and deductibles are collected at time of service.
- Phone & fax numbers for your referring doctor and primary care physician.
If you are unable to make your scheduled appointment, we request that you call our office with a 24-hour notice of the cancellation.
